How to use the Prevention System Gap Score Assessment

The purpose of the assessment is to identify strengths as well as priority areas for continued growth and development.  The Prevention System Assessment tool uses a “Gap Analysis” to help coalitions prioritize areas for development by measuring the discrepancies—or “gaps”—between their current stage of development for a number of indicators and the stage of development they feel is needed to be most effective. 

Instructions:

 ■ Please enter the current date and select your current coalition affiliation.  If you serve on more than one coalition, you will need to complete the assessment for each coalition.

 ■ Please rate each question on a scale of 1 to 10.  1 is strongly disagree and 10 is strongly agree.  GAP analysis reports will be given to each coalition.

 ■ All boxes must be checked before you send the form.

 ■ When you are finished with the analysis, click the submit button.

 ■ You will receive a confirmation page.  Please print the confirmation page and bring it to your next coalition meeting.

Here’s an example:

As an applicant, one of the indicators you will rate concerns strategic planning by the prevention system’s leadership.  It reads:

“Prevention is identified as a priority program area and funded accordingly in the budgets of the prevention system member organizations and agencies.”

It may be that this statement is not at all true of your local coalition at this time. On a scale of 1 to 10, you rate this statement a “1” because coalition members strongly disagree that it is representative of your local coalition as it currently exists.  But coalition members give that same statement a priority rating of “9,” because they consider raising the indicator rating to be a high priority for the local coalition in order for it to be successful at achieving its goals, objectives and outcomes.  The difference between your assessment rating (1) from your priority rating (9) illustrates the gap between where your local coalition is today and where coalition members feel it needs to be in order to maximize its effectiveness. 

The gap analysis that results from this exercise illustrates important areas of change for each coalition.  Please complete the analysis and submit.  All results are auto tabulated and will be used for planning with the regional and local LiveWise coalitions.
