Integrated Care Coordination Unit

The Integrated Care Coordination Unit (ICCU) is a collaboration between
Region 6 Behavioral Healthcare, Nebraska Department of Health and Human Services, and Nebraska Family Support Network serving state ward youth and their families.  The ICCU provides family advocacy, protection and safety, and case management in a team-based approach for out-of-home state ward children and their families.

Program Elements
The ICCU strives to involve the family at every level of programming.  A family advocate is available to the family to assist in the process of transitioning the child to his or her home.  As well, a Care Coordinator acts as the case manager and facilitates the child and family team meetings in order to better help the parents to increase the family strengths.  Together the family, Family Advocate, and Care Coordinator work as a team to ensure the success of the child and family.  

How It Works
Youth  and their families are referred to the ICCU by the Department of Health and Human Services.  An ICCU Care Coordinator then works with the family providing case management using wraparound principles.  Wraparound principles incorporate a family team approach which enhances youth and family strengths, utilizes community supports, and builds independence
as the family strives to reunite.
