The Region 6 Professional Partner Program utilizes a wraparound philosophy to assist children experiencing behavioral or emotional concerns by identifying and coordinating community-based resources.
Program Elements ■ Child and family-focused services ■ Working partnership between family and identified team members ■ Strength-based ■ Individualized treatment plansHow It Works Each family enrolled in the program is assigned a case manager who is familiar with available community resources and experienced in working with youth and their families. In partnership with the family, the case manager assists with the development of an Individualized Family Service Plan. This plan may include traditional mental health services, community resources, and most importantly, informal supports.
Program Participants ■ Families with children, ages birth to 21 years old, who have behavioral or emotional concerns ■ Parents who may feel hopeless and are not sure where to turn for help ■ Parents who have tried to find help but were unable to find services they could afford ■ Youth who may be at risk of being suspended from school or being removed from their home ■ Parents who wish to have an active part in developing a plan to address the needs of the the identified child and the family
Cost and Eligibility There is no cost to participate in the Professional Partner program. Families who are residents of Cass, Dodge, Douglas, Sarpy, or Washington counties and who have a child/children struggling with behavioral or emotional concerns are eligible to participate in the program.
