The program is designed for adults living with a chronic condition and/or caring for someone with a chronic condition\, such as heart disease\, diabetes\, cancer\, depression\, chronic pain\, arthritis\, or any other chronic health concern(s). \nLearn how to manage symptoms\, organize medications\, cope with emotions\, eat healthily\, manage weight\, and communicate with family\, friends and healthcare professionals. \nCome join us! The cost is FREE and includes program materials. \nRegistration is required as class size is limited.
