PROFESSIONAL PARTNER PROGRAM
Team-Based Support
The Professional Partner Program supports youth, young adults and families working through complex behavioral and mental health needs using a team-based process called Wraparound.
WHAT IS WRAPAROUND?
Wraparound is a collaborative, team-based approach that unites youth, young adults, families and the people they trust to develop solutions and work toward a common vision for success.
The Wraparound process is guided by a trained facilitator who helps the team create and follow a plan that supports success at home, in school and in the community. Families are active partners and decision-makers in every step of the process.
Region 6 does not provide counseling or therapy through this program. Instead, our Professional Partners facilitate the Wraparound process, bringing families, providers, schools, natural supports and community resources together around one shared plan. Our role is to partner with the family team and empower parents to be in the driver’s seat of their children’s services.
WHAT TO EXPECT FROM THE PROFESSIONAL PARTNER PROGRAM
Wraparound is a four-step process: Engagement and Team Preparation, Initial Plan Development, Plan Implementation and Transition.
Through working with the Professional Partner Program, families can expect to:
- Participate in monthly team meetings
- Build a team of people involved in their child’s life
- Identify their unique family strengths, needs and goals
- Create and implement a plan that uses natural supports and community resources to navigate challenges and crises
- Support transition from services to sustain family success
PROFESSIONAL PARTNER PROGRAM TRACKS
TRADITIONAL
Ages 3–21
Referrals can come from anywhere: the person served, family members, providers, other community supports, etc.
This track focuses on improving youth functioning without the need for out-of-home placement.
RAPID RESPONSE
Ages 3–19
Referrals come from the County Attorney, hospitals and Mobile Crisis Response.
This track focuses on stabilizing the youth and home environment to minimize involvement in child welfare and juvenile justice systems.
TRANSITION
Ages 16–25
Referrals can come from anywhere: the person served, family members, providers, other community supports, etc.
This track focuses on developing supports and services that help individuals successfully transition into adulthood.
