TRANSITION AGE Professional Partner PROGRAM
The Professional Partner Program is a care coordination program that supports youth, young adults and families working through complex behavioral and mental health needs using a team-based process called Wraparound. This track focuses on developing supports and services that help individuals successfully transition into adulthood.
